Forms & Templates

We want your experience as a Step Up supervisor to be a great one! Below you can find a variety of forms and templates to help you throughout the summer and our Level 2 Supervisor Forms Manual outlines how to use each resource.

Additional Resources

These resources will help you address situations youth may be facing. You may encounter a mental health crisis or a youth struggling with homelessness. These resources will guide you in how to approach such situations, know what resources are available and help you be an advocate for our youth. The Supervisor Resources Guide further explains what you will find in these resources to ease your navigation and resource identification.
